W16.221 is 'Fall in (into) bucket of water causing drowning and submersion,' and it is one of the codes people screenshot as proof that ICD-10-CM has lost its mind. It has not. A bucket of water is a documented pediatric drowning hazard - a toddler is top-heavy, goes in head first, and cannot push back out of a straight-sided pail. The code set gave it its own subcategory because public health needed to count it. For coders it is also a clean lesson in the three rules that govern all of Chapter 20: the external cause code is never first, it needs a 7th character, and the sixth digit is doing real work.
